22VAC40-73 standard 22VAC40-73-1090-A: Based on record review and interview, the facility did not ensure that prior to each resident?s admission to the safe, secure environment, that the resident is assessed by an independent clinical psychologist as having a serious cognitive impairment due to a primary psychiatric diagnosis of dementia with an inability to recognize danger or protect his own safety and welfare.

Resident #1 has been re-evaluated by provider, and a new Assessment of Serious Cognitive Impairment was added to Resident #1's records indicating Resident #1 is not able to recognize danger or protect his own safety and welfare. An audit of the Assessment of Serious Cognitive Impairment of residents currently residing in the safe, secure environment was performed to ensure accuracy. Any incomplete or inaccurate forms will be scheduled for a provider re-assessment. The Medical Director has started re-education with providers on the regulatory requirement to complete the Assessment of Serious Cognitive Impairment accurately. Date of compliance 10.01.2025.
