As part of the intervention right after the incident facility initiated a POC that included: Elopement training was completed on 10/9/2025 with staff members. Facility performed 3 elopement drills (1 per shift). An audit of all Memory Care Residents was completed on 10/15/2025 to ensure that any resident at risk for elopement and/or exit seeking/exploring have been identified and appropriate resident specific interventions are in place. Weekly audits were performed during November, and no additional residents were identified at risk. A monthly audit will continue for 2 months to ensure all new admits have been assessed for safety and exploring/elopement risk and appropriate resident specific interventions have been implemented. Additional Elopement drills will be conducted monthly for 90 days to ensure compliance and understanding of education. Completion date 1/15/2026. These audits and drills will be reported to the QAPI committee monthly for 90 days for further review, recommendations, feedback and additional audits.
